Register


Application materials consist of the following documents:

  • Enrollment form
  • Emergency medical care authorization
  • Parent and Participant Statement of Agreement
  • Payment method and t-shirt order form
All of these documents are included in the registration packet linked below, along with details on how to fill it out.  For the digital form, download it from the page that you are directed to, fill out the forms, save a copy, and e-mail it to percyjacksoncamp@gmail.com.  Keep in mind that some parts of the packet MUST be physcially delivered to Camp Half-Blood Elmhurst Branch.  For the printable version, simply print the entire document, fill in the blanks, and mail or deliver in person to camp headquarters, as directed by the packet.

    is it possible to fill out the forms on the computer with the link above? if so, HOW???

      Hi Confused,
      For now, no, you can only fill out the forms by printing them first. We are working on putting up a separate link soon for online registration, which will be available later this week. Thanks for asking!
